Two Things I learned about Healthcare Excellence Canada 

1. Supports National Quality Improvement Initiative 

Through exploring Healthcare Excellence Canada, I learned that HEC leads and supports large scale national quality improvement initiatives SUCH AS THE Right Care Challenges and Enhancing Integrated Care programs (Healthcare Excellence Canada [HEC], 2026). These initiatives help healthcare organizations apply structured improvement methodologies to enhance patient outcomes, strengthen care coordination, and reduce system pressures. 

What stood out to me personally is how these programs move beyond theory and focus on practical, measurable improvements in real healthcare setting. As a practical nursing student, I often focus on direct bedside care. However, learning about HEC helped me understand that quality improvement also occurs at the system level. It made me realize that even as an LPN, I contribute to broader healthcare improvements by participating in adults, reporting safety concerns, and supporting evidence- based changes in practice. Understanding HEC’s role expanded my perspective from individual patient care to system- wide impact (HEC, 2026). 

2. Promoting integrated, person- centred care 

Second significant learning is that HEC strongly promotes integrated, person- centred care models that strengthen collaboration between primary care, community services, and social providers (HEC, 2026). This approach aims to reduce fragmentation in healthcare delivery and endure that patients experience continuity of care across settings. 

This learning resonated with me because older adults often transition between hospital, community, and long-term care environments. I have observed during clinical experiences how gaps in communication can negatively affect seniors’ outcomes. Learning about HEC’s emphasis on integrated care reinforced the importance of teamwork, communication, and coordinated discharge planning. It deepened my understanding that quality gerontological nursing care is not only about performing clinical tasks but also about ensuring safe transitions and continuity of care (HEC,2026). 

This insight has strengthened my commitment to collaborating effectively withing interdisciplinary teams and advocating foe seamless care for older adults.
